Just installed it on 1.5.4, and having the same problem. It seems to save *some* of the changes made but not all, and also leaves blank boxes around. All very confusing. Also the details suggest you can change the ad size from the default of 32x32 but I can't find where to do it.

Not sure if this helps or not, but regarding the image size, I just created a new blank image that was 16 x 16 px. I've attached a copy in case you can use it.

There are no parameters to change the size. All you have to do is edit your jpg to the size you want. Ftp your new image to the appropriate folder. Then add your text to the position of your choosing.
If you don't want any blank images, all you have to do is click No to any of yes enabled positions and save.
